Medication Consultation Services
The definitive goal of pharmacists providing medication consultation services is to improve patients’ overall health outcomes. The Framework to Support Comprehensive Medication Consultation Services has been developed to support and guide pharmacists who currently provide medication consultation services in their practice and those who are looking to provide quality medication consultation services to their patients. This document is founded upon, and intended to compliment the College’s Standards of Practice.
In order to determine and prioritize short and long-term goals, a survey was developed and presented to pharmacists at the 2004 District Meetings. One of the goals of the District Meetings was to assess how members envisioned themselves optimizing and advancing their practice in the interest of the public.
Results of the survey and meetings reported in the September/October 2004 issue of Pharmacy Connection indicated that conducting medication reviews was a practice most likely to be readily adopted by pharmacists. In fact, some pharmacists reported that they had already begun to offer some type of medication reviews to their patients as an added service.
The Task Force on Optimizing the Pharmacist’s Role recommended to Council the development of guidelines that would provide a framework to support consistent and safe delivery of medication reviews. In September of 2004, Council appointed the Task Force to put this recommendation into action.
The Task Force has taken member’s feedback and has developed this Framework as the first step in optimizing the role of the pharmacist. The Task Force is now working on other projects such as collaborative practice models and other initiatives to encourage for the evolution of the profession and advancement of primary health care.
