Letter of Intent (click here for sample) must be received by Client Services at least 30 days prior to the proposed opening date.

If new opening is approved, Client Services will respond with a confirmation letter and will include:
- Opening a New Pharmacy checklist (this is for your reference and does not have to be returned)
- Application for Certificate of Accreditation of a Pharmacy (Form 3)
- Narcotic Signing Authority Form
- Corporation Information Form
- Copy of the “Required Reference Guide for Pharmacies in Ontario”
Articles of Incorporation and Share Certificates must be submitted to OCP along with the above OCP forms. For further information please click here to refer to the Ministry of Government Services website.
A copy of the confirmation letter is also sent to any other directors listed in the Letter of Intent.
Once ALL OCP forms, Articles of Incorporation and the $1050 fee have been received and are in order, Client Services will contact an OCP Field Representative and advise him/her of the date on which the pharmacy will be ready for inspection and the proposed opening date.
Client Services will contact the new owner and:
- Confirm that all is in order
- Inform new owner that a Field Rep will be in contact
- Ask the new owner how they would like to receive the Opening Kit (mail or accessed online)
- If a pharmacy opens on or before May 10th, in addition to the new opening fee of $1050, the annual pharmacy accreditation fee will be due on May 10.
Inspection
OCP Field Rep will inform Client Services when a pharmacy has been accredited after inspection.
Client Services will inform the Ontario Drug Benefit (ODB) that the pharmacy has been accredited.
Please note: ODB is not open on weekends therefore all new openings must occur on a weekday.
Opening Day
Client Services will call the pharmacy on opening day to confirm:
- Pharmacy is now open
- Who the Staff are who will be working in the pharmacy
Once confirmed, a Certificate of Accreditation and receipt will be mailed to the pharmacy.
