Purchasing a Pharmacy


Letter of Intent (click here to open and print the form) must be received by Client Services at least 15 days prior to the acquisition of the pharmacy.

Client Services responds with a confirmation letter and includes:
- Application for Certificate of Accreditation of a Pharmacy (Form 3)
- Narcotic Signing Authority Form
- Corporation Information Form

Articles of Incorporation and Share Certificates must be submitted to OCP along with the above OCP forms. For further information refer to the Ministry of Government Services website: http://www.cbs.gov.on.ca/mcbs/english/5httcp.htm

Completed forms, the $525 fee and the Articles of Incorporation must be received by Client Services at least 7 days prior to the acquisition of the pharmacy.

Please note: If the above forms and fee are not received on time, the Ontario Drug Benefit (ODB) closes the account of the current associate on the acquisition date.
ODB is not open on weekends therefore all acquisitions must occur on a weekday.

Once the acquisition is complete, Client Services informs the OCP Field Rep; the OCP Field Rep will conduct an inspection at a time in accordance with their schedule.
Client Services will then forward a new Certificate of Accreditation reflecting the new corporation name. The accreditation number of a pharmacy does not change upon acquisition.
