Relocating a Pharmacy


Letter of Intent (Click here for sample ) must be received by Client Services at least 30 days prior to relocation date

Client Services will respond with a confirmation letter and will include:
- Application for Certificate of Accreditation of a Pharmacy (Form 3)
- Narcotic Signing Authority From
- Copy of the “Required Reference Guide for Pharmacies in Ontario”

Completed forms, and the $525 fee must be received by Client Services at least 7 days prior to the relocation of the pharmacy

Once the relocation is complete, Client Services will inform the OCP Field Rep; the OCP field Rep will conduct an inspection at a time in accordance with their schedule.
Client Services will then forward a new Certificate of Accreditation reflecting the change of address. The accreditation number of a pharmacy does not change with a relocation.
