Focus on Error Prevention: New Drug Therapy

It’s critical that steps be taken to prevent the dispensing of drugs that patients should no longer be taking. In this “Focus on Error Prevention” article, we outline a case that’s a great learning opportunity and provide five important recommendations to avoid dispensing errors. Some of these recommendations include:

  • Remind staff to always confirm the name of the medication the patient is requesting when processing refills
  • Whenever a new medication is added to the pharmacy inventory, all staff should be educated about the product

CAMH’s Pharmacist’s Guide: Opioid Agonist Maintenance Treatment

CAMH Pharmacist's Guide

This is a reminder that in September 2015, the Centre for Addiction and Mental Health (CAMH) released their latest pharmacist’s guide called Opioid Agonist Maintenance Treatment: A Pharmacist’s Guide to Methadone and Buprenorphine for Opioid Use Disorder. This third edition replaces all previous editions of Methadone Maintenance: A Pharmacist’s Guide to Treatment.

This is an essential manual for all pharmacists involved in opioid agonist maintenance treatment (OAMT) and a required reference for Ontario pharmacies that dispense methadone. All pharmacies that dispense methadone must have a copy of the most recent edition of this reference.
