Diabetes is becoming more and more prevalent in society and pharmacists have the opportunity to have a significant impact on the health and well-being of their patients who are living with this disease. To have the greatest possible impact on patient care, its important that pharmacists have the best tools and knowledge to support their patients. The Ontario Pharmacists Associations Master Plan for Diabetes Continuing Education program is designed to support those pharmacists who have an interest in furthering their knowledge in the foundations of diabetes.
This new online professional development program is taught by experts in diabetes care and puts a strong emphasis on providing education for the self-management of diabetes. A variety of topics are covered, including: motivational interviewing, pharmacological management of diabetes, self-monitoring of blood glucose, complications, managing special circumstances, nutrition, and exercise.
Program participants will have access to an interactive tool that includes mandatory Ministry of Health and Long-Term Care (MOHLTC) documentation forms, 27 separate forms containing patient teaching points for various areas of diabetes that have been developed to facilitate effective interactions with patients, reference resources for healthcare professionals, and resources that can be provided to patients with diabetes.
