Incident Analysis and Proactive Risk Assessment

Summary

This workshop provides healthcare practitioners with background theory and hands-on practice in incident analysis (Root Cause Analysis, RCA) and proactive risk assessment using Failure Mode and Effects Analysis (FMEA).

The live virtual workshop is most appropriate for:
– Supervisors, designated managers and owners involved incidents resulting in any level of harm. The live workshop uses a simulation approach to support participants to analyze and problem solve why an incident occurred, and to brainstorm and develop strategies to prevent a similar incident from occurring in the future. The team environment mimics a real life analysis, and an expert facilitator is available to guide them through the process of analyzing a case (day 1) and to practicing being proactive in preventing harm (day 2).
– Staff pharmacists and pharmacy technicians involved in incidents causing moderate to severe harm, to provide more in-depth learning to support their understanding and application of the RCA and FMEA processes.

Note: For the new virtual RCA/FMEA workshop, the website shows the combined price of day 1 (RCA) and day 2 (FMEA), however, if a participant would like to only register for day 1, they can do so by emailing education@ismpcanada.ca. We still offer day 1 (RCA) registration as well at a lower price. We have also included education about the AIMs program into the workshop (on Day 1) as well.
