As outlined in your prior notice letter, all documentation should be submitted two weeks ahead of your scheduled hospital operations assessment date. This allows the operations advisor enough time to review thoroughly and to contact you for clarification or missing documents.

In order to ensure that the operations advisor knows which documents correspond to which type of activity, we ask that you please NAME the document attachments to indicate this clearly. For example: “BSC Certification Reports for the Oncology Center” or “Health Canada Narcotic Loss Report Aug 2020” or “Feb 2019 Narcotic Audit of Unit

The type, and amount, of documents we are asking for will be clearly laid out in the prior notice letter sent by your operations advisor. Please review this carefully. This will differ depending on whether your site completes sterile compounding or not. The purpose of asking for a variety of different documents is so the operations advisor is able to get a good picture of the breadth of your operations and the types of services you manage. The most important thing to consider is that we are trying to understand your policies and procedures.

Please send the most recent documents to allow us to ask you questions about how you optimize care for your patients. We realize that every hospital is unique, so if there are any special services that you provide, or specific cases you would like to share, please let your operation advisor know and be sure to include these. If there is any reason you cannot provide certain types of documentation, please also discuss with your operations advisor.

If your hospital completes sterile compounding, for each environmental monitoring report that is sent, please also include the quality assurance component, outlined in section 7. Quality Assurance Program & 7.2 Results and Action Levels in the relevant NAPRA Model Standards for Pharmacy Compounding document, for ease of discussion and review.

As outlined in your prior notice letter, as the designated contact, you are required to give an update in Quill regarding items that were outside of compliance during the last assessment. An update must be completed two weeks ahead of your scheduled hospital operations assessment date. This allows the operations advisor enough time to review thoroughly and to contact you for clarification. When responding to each item, please include a timeline for anticipated completion for all criteria, as well as the current status of the criterion.
