Update on the College’s Initiative Regarding Mandating Third Party Medication Incident Reporting

At the December 2016 College Council meeting, Council was unanimous in its support of requiring the reporting of medication incidents to an external third party. The expectation is that aggregate analyses of medication incidents will be received by the College for targeted practice improvement initiatives, which in turn will help improve patient safety.

To this end, the College has established a task force to examine this subject and develop a model for the implementation of incident reporting, for review and consultation. This task force is composed of pharmacy professionals and a patient representative. An update from the task force on their work is expected at the March 2017 Council meeting.

The College is committed to continuous quality improvement and transparency. We look forward to working with government, pharmacy professionals and stakeholders to enhance the safe, effective and ethical delivery of pharmacy services in Ontario.
