The College should be notified when there is any material change to the size or physical layout of an existing accredited hospital inpatient primary pharmacy location, or if the hospital inpatient primary pharmacy location is moving to a new location at the same municipal address.
Please email the assigned hospital operations advisor 30 days prior to commencing any renovations to a pharmacy.
The College’s Hospital Operations Advisor will review the proposed changes to the hospital inpatient primary pharmacy location and connect with the Hospital Designated Contact if they have any questions or concerns. If the hospital is continuing or commencing sterile compounding services, an onsite assessment will be required. The Hospital Operations Advisor will contact the Hospital’s Designated Contact to schedule an assessment date.
